Chloe Cole Urges Senate to Ban Gender-Affirming Care for Minors

A contentious Senate hearing on gender-affirming care for transgender minors placed detransitioner and activist Chloe Cole at the center of a growing national debate over who should make healthcare decisions for transgender youth.

The June 3 hearing before the Senate Health, Education, Labor and Pensions Committee was convened by Republican lawmakers amid ongoing efforts by the Trump administration to restrict access to gender-affirming care nationwide. While multiple witnesses testified, much of the discussion centered on Cole’s personal experience transitioning as a minor and later detransitioning.

Cole, now 21, told senators she began medically transitioning at age 12, underwent a double mastectomy at 15, and detransitioned a year later. During her testimony, she argued that minors cannot fully understand the long-term consequences of medical transition and called for federal legislation banning puberty blockers, hormone therapy, and gender-affirming surgeries for minors.

“This is medical abuse,” Cole told lawmakers when asked whether Congress should enact nationwide restrictions. She also described feeling inadequately informed about potential long-term effects, including fertility concerns and the loss of her ability to breastfeed.

Republican committee members, including Sen. Bill Cassidy, highlighted Cole’s story as evidence that stronger federal oversight is needed. Cassidy argued that children are increasingly being influenced toward medical transition and framed the issue as one of patient safety.

However, Democrats and LGBTQ advocates challenged the idea that one individual’s experience should determine healthcare access for all transgender youth. Witness Shannon Minter argued that gender-affirming care is governed by established medical guidelines that involve extensive evaluation, informed consent, and parental participation. He maintained that decisions should remain between families, patients, and healthcare providers rather than politicians.

The hearing also highlighted a significant divide between political rhetoric and medical guidance. Major medical organizations, including the American Medical Association and the American Academy of Pediatrics, continue to support access to gender-affirming care for transgender youth while reviewing and updating clinical recommendations as new research emerges. These organizations maintain that treatment decisions should be individualized and made by patients, families, and healthcare providers rather than politicians.

Research discussed during the hearing showed that gender-affirming medical treatment for minors remains relatively uncommon, with surgeries especially rare among transgender adolescents. Opponents of restrictions pointed to these findings as evidence that existing safeguards are already limiting access to carefully evaluated cases.

As lawmakers continue debating the future of transgender healthcare, Cole’s testimony is likely to remain a central talking point for advocates seeking nationwide restrictions. At the same time, transgender rights organizations warn that policy decisions based primarily on rare detransition experiences risk overshadowing the needs and experiences of the vast majority of transgender youth who continue to seek and benefit from gender-affirming care.

The hearing underscores how transgender healthcare remains one of the most politically charged issues in the United States. While Cole’s story has become a powerful tool for opponents of gender-affirming care, supporters argue that public policy should reflect the experiences of the broader transgender population and the medical professionals who treat them rather than relying on individual cases alone.
