Washington Wants Genital Exams to Keep Trans Girls Out

They claim they're protecting girls. Their solution? A Washington ballot measure that could subject schoolgirls to genital exams to keep a handful of trans athletes out of sports. Apparently, protecting girls' privacy now means invading it.

A Washington ballot initiative targeting transgender student athletes could force every girl participating in school sports to undergo medical sex verification, drawing fierce opposition from physicians, educators, and LGBTQ advocates ahead of the November 3 election.

Initiative 638, backed by conservative political action committee Let’s Go Washington, would prohibit transgender girls from competing on girls’ school sports teams. However, the proposal goes considerably further than simply restricting transgender participation. It would require all girls seeking to participate in designated female athletic competitions to provide medical documentation verifying their biological sex.

Under the proposed law, healthcare providers would determine eligibility using reproductive anatomy, genetic information, or naturally produced testosterone levels. Critics warn that complying could involve genital examinations, blood tests, or genetic screening, even for girls who have never had their sex questioned.

Doctors Push Back Against Invasive Requirements

According to reporting, more than 140 healthcare professionals, nurses, and educators have raised concerns about the measure, describing its potential consequences as invasive and medically unnecessary.

The proposal’s supporters argue that separating athletes by biological sex protects competitive fairness, safety, and opportunities for female students.

But medical professionals dispute the suggestion that the required verification is already part of routine sports physicals. Current examinations primarily assess cardiovascular health, vision, breathing, and other factors affecting athletic safety. They do not routinely require genital examinations, chromosome testing, or hormone analysis.

Hundreds of Thousands Affected, Fewer Than 10 Trans Athletes

Perhaps the most striking detail involves the number of students potentially affected.

The Washington Interscholastic Activities Association oversees sports involving more than 200,000 students statewide. Yet the organization reports awareness of approximately five to ten transgender athletes participating in school sports.

That means a policy intended to exclude a handful of transgender girls could impose additional medical verification requirements on every girl seeking to participate in girls’ athletics.

November Vote Could Reshape School Sports

The initiative’s principal financial backer is multimillionaire Brian Heywood, whose Let’s Go Washington organization has championed several conservative ballot measures.

Opponents, including Governor Bob Ferguson and Senator Patty Murray, argue that the proposal threatens student privacy and unnecessarily targets transgender children.

Despite Washington’s progressive reputation, the outcome remains uncertain. An August poll commissioned by Let’s Go Washington reportedly found 56% support, although its full methodology and results have not been publicly released.

Voters will decide the measure on November 3.

For transgender students, the referendum could determine whether they can continue playing alongside their classmates. For every other girl participating in school sports, it raises another question: Should access to a school athletic team require medical verification of a child’s sex?
